2022 Chapter Membership Forms
Chapter Membership Enrollment Form and Release for Adults - English / French. Each individual wishing to enroll in the Chapter must complete an Enrollment form! Completed forms can be left with the Dealership or given to a member of the Chapter Executive for processing.
NOTE: Annual Dues - to accompany Enrollment Form(s) are $20.00 Single or $25.00 Family.

H.O.G.® Canada has a zero alcohol tolerance policy. All Chapter members shall refrain from consuming alcohol during official Chapter riding events. Any Chapter member that chooses to ignore this policy, shall be subject to the immediate removal of his/her participation in the event and shall not be eligible for any prizes awarded during the official portion of such event.
